ALL patient charts will be stored at our Fort Madison office. The office is attached to Fort Madison Community Hospital on the west end of Fort Madison along Highway 61.

PATIENTS WHO RECEIVE ALLERGY SHOTS We will continue to mix allergy immunization extracts as before. You may receive your actual injections in our Fort Madison office or you may pick up your extract and take it to your primary care physician who may administer the shot in their office. Our Fort Madison office is open from 8 until 12 and from 1 until 5. We are also open every five or six weeks on a Saturday morning.

Burlington patients may begin to make appointments now for our Fort Madison office by calling 319 372 6280.

We would be delighted if all of our Burlington patients follow us in our move to Fort Madison, but we understand this may not be possible for everyone. For those who wish records to be transferred elsewhere, we will help you accomplish this as smoothly as possible following the proper execution of a standard record release form.
